Those small dark gaps between your teeth at the gumline have a name, a cause, and a fix. Most black triangles can be closed in a single visit with no drilling.
What black triangles are
Look closely at a healthy smile and you will notice that the gum tissue rises to a small point between every pair of teeth. That point is called the papilla, and it fills the space between the teeth right down to where they touch. When the papilla shrinks, or when the teeth are shaped so that they touch only near the biting edge, a small dark gap opens up between the teeth at the gumline. Because the mouth behind it is dark, the gap looks black, and because of its shape it is called a black triangle. Dentists call it an open gingival embrasure.
Black triangles are extremely common and most people who have them notice them long before anyone else does. They are most visible between the upper front teeth, where they can make an otherwise straight, white smile look older than it is. They are also more than cosmetic. Food packs into them, saliva can spray through them when you speak, and the exposed root surfaces at the edges of the gap are more prone to sensitivity and decay.
What causes black triangles
There is usually more than one factor at work, and identifying which ones apply to you determines the right treatment.
- Gum recession. Aggressive brushing, thin gum tissue, and age all pull the gumline down and shrink the papilla.
- Bone loss from periodontal disease. The papilla sits on top of the bone between the teeth. When gum disease erodes that bone, the papilla loses its support and flattens. This is the cause we take most seriously, because it is progressive until it is treated.
- Tooth shape. Some teeth are naturally triangular, wide at the biting edge and narrow at the neck. Two triangular teeth side by side touch only at the top and leave a gap below no matter how healthy the gums are.
- Orthodontic treatment. Straightening crowded teeth often reveals black triangles that were hidden when the teeth overlapped. This is one of the most common reasons adults ask about them after finishing braces or Invisalign.
- Previous dental work. Fillings or crowns with the wrong contour can leave a space the gum cannot fill.
- Age. The papilla naturally loses height over the decades, which is why black triangles are read subconsciously as a sign of an aging smile.
Why they are worth treating
Black triangles are not dangerous on their own, and plenty of patients choose to leave them. The reasons people decide to close them tend to fall into three groups. The first is appearance: a row of dark gaps draws the eye and ages the smile more than most people realize until they see the after photo. The second is function: food trapping between the front teeth after every meal is a daily irritation, and the whistling or spitting some patients notice when they talk is directly related to air and saliva moving through the gaps. The third is health: exposed root surfaces at the edges of a black triangle are softer than enamel, more sensitive, and more prone to decay, and food packing keeps the gum in that area chronically inflamed.
The one situation where treatment is not optional is when the black triangles are a symptom of active periodontal disease. Composite bonding for black triangles, step by step
- Evaluation. We measure the gaps, check the health of the gum and bone with X-rays, and confirm that periodontal disease is not the cause. Photos are taken for planning.
- Shade match. The composite is matched to your teeth, or to your target shade if you are whitening first.
- Isolation and conditioning. The teeth are kept dry and the enamel is conditioned with an etching gel and bonding agent. Nothing is drilled.
- Shaping the new contour. Using thin anatomic matrices that hug the tooth below the gumline, composite is added in layers to rebuild the natural curve of the tooth from the gum up to the contact point. Each layer is cured with a blue light.
- Finishing. The new contours are refined so floss passes cleanly, the bite is checked, and the surfaces are polished to match the surrounding enamel.
- Follow up. The papilla often fills in further over the next few months now that it has the right shape to support it. We check the area at your next cleaning.
A pair of front teeth is typically finished in under an hour. Closing black triangles across all six upper front teeth is a longer single visit.
Who is a good candidate
Composite closure works best when the gums are healthy, the teeth are stable, and the gaps are small to moderate. Larger gaps can still be improved, though the result may involve a compromise between closing the space fully and keeping the teeth from looking too wide. Patients with active gum disease need periodontal treatment first. Patients with significant crowding or rotation are often better served by clear aligner treatment before any bonding, since straightening changes where the teeth touch.
